FABP is introduced as a novel plasma marker for the detection of acute myocardial infarction (AMI). The kinetics of plasma FABP (15kD) concentration is closely related to the onset of AMI, rising significantly after 2 hours and returning to normal after 18-24 hours. The concentration of FABP in skeletal muscle is 20 times lower than in myocardium (myoglobin levels are similar in myocardium and skeletal muscle), making FABP more myocardial specific than myoglobin. This makes it myocardial specific to Myoglobin. Accordingly, FABP is a useful biochemical marker to prove or reject the presence of AMI. FABP levels can also be used as a useful plasma marker to determine the size of a myocardial infarction. FABP can be used as a standard in biochemical and immunochemical studies as well as a mass standard for antiserum FABP in methods used for early detection of acute myocardial infarction.
Myocardial Troponin I (cTnI) is a protein of 22.5 kD molecular weight found in myocardium. Troponin I consists of three parts: troponin I, troponin T and troponin C complexes. Together with tropomyosin, this structural complex is the major regulator of actomyosin calcium-sensitive ATPase activity in cardiac and striated muscle. Troponin I is released and released into the blood 4-6 hours after the onset of pain due to myocardial injury. The release process of Troponin I and CK-MB is similar, but while CK-MB levels decrease to normal within 72 hours, Troponin I levels remain elevated for 6-10 days, giving a longer period of time to detect myocardial injury. cTnI specifically detects muscle injury as seen in perioperative injury, marathon running or chest trauma.
